Our office proudly uses Unified Practice software, a system designed specially for Chinese medicine providers that maintains electronic health records in accordance with California law and HIPPA regulations.
New patients will receive a confirmation email and a link to Onboarding Forms. Please take time to carefully complete the health intake online, read and sign the consent forms in order for your provider to review your health history prior to your first visit.
If you cannot complete the forms online beforehand, we request you arrive 15 minutes early to complete them on our iPad.
